11/29/2023 EnchantaVaughn Approved Behavior: Client was reported to have displayed hyperactive behaviors as he targeted siblings to draw attention as parent shared that she directed to stop the behaviors. Client shared difficulty with compliance as he reportedly continued to show hyperactive behaviors with behaviors lasting 2-3 times daily for 20 minutes in duration. Behavior: Client self-reported to QMHP on how struggles with managing conflict with others as he will fight his siblings on a daily basis and scream to the top of his lungs with behaviors lasting 20 minutes in duration. Intervention: QMHP advised client to apologize to his family for displaying agitating behaviors towards siblings. QMHP instructed client to participate with role play exercise. QMHP explained that the role play exercise will assist him to identify his behaviors and moods. Intervention: QMHP played the role of the client as client to observe in review the behavior of hyperactivity and provoking behavior. QMHP continued the session by asking client about the use of problem solving exercises. QMHP reviewed problem solving scenario for the client can think and utilize the important decisions that are relevant to reducing conflicts within the home. Parent was encouraged to place reminders for the client to make positive decisions. Response: Client was able to show participation with the exercises as he he was able to address his understanding of how he can utilize the role play exercise. Client was able to apologize to his siblings as he shared that he was only having fun. Response: Client's participation with the problem solving exercise showed that he was able to identify social skills needed to avoid conflicts. Parent agreed to utilize verbal reminders to have client follow directives and positive decisions.
